All appropriate vaccines and dewormings for the puppy’s age have been given, and a record of such has been supplied to Buyer at the time of purchase (see attached vaccination certificate from the Veterinarian). Vaccinations given: D, H, P&P on __________. Worming-Nemex 2 on: ___________,___________ and ____________.

In purchasing this puppy described above for a( family pet or breeder dog circle one), I agree to the following as indicated below:

1. Buyer will provide a safe, indoor home environment for puppy, take good care of puppy and provide adequate food and annual routine preventive care by a licensed veterinarian. Buyer also agrees to keep the puppy properly socialized and to complete at least one obedience class.

2. The Buyer has agreed to have the puppy spayed or neutered between 6-9 months of age unless buying for breeder dog. and to

keep puppy properly exercised to maintain a healthy weight. Buyer also agreed to NOT over feed

and cause the puppy to become overweight at anytime in the puppy’s life.

3. Puppy shall not be wholesaled, retailed or sold to any party, pet store, pet broker etc.

4. Buyer agrees to return the puppy/dog to the breeder if it becomes necessary for any reason for buyer to relinquish ownership. Puppy/dog must be returned in the same condition (SOUND HEALTH & TEMPERAMENT), it was when acquired along with the AKC papers. I will try to place a good home for your puppy any money received I will send to you!

5. Under NO circumstances is the above puppy/dog to be chained outside. If the above pup/dog is to be left outside unsupervised than a kennel run or a fenced in yard must be constructed.

6. The breeder guarantees that this puppy to be of sound health (free from illness and defects) and temperament at time of sale. You must visit a licensed veterinarian within 10 days after you obtain possession of puppy (not including holidays and weekends). Should the puppy be determined to be in ill health, with a letter from the Veterinarian stating so, you agree to notify us with in 2 business days from the Veterinarian exam, and return puppy to us, the purchase price will be refunded (excluding the puppy deposit if any), or you will receive a replacement puppy from the next available litter in the same pick order. Breeder also reserves the right to have the puppy examined by a Veterinarian of the breeder’s choice to confirm the diagnosis.

7. Precautions have been taken to prevent this puppy from acquiring problems common for the breed, such as hip dysplasia or heart and eye conditions as determined by the Golden Retriever Club of America. However, due to complex genetic interactions and environmental factors beyond my control, I can not guarantee that this puppy will be free of genetic disorders. In the event that the puppy/dog is determined to be severely dysplastic or have severe heart or eye conditions up to 12 months of age, seller will refund the purchase price (excluding the puppy deposit if any), or give a replacement puppy from the next available litter in the same pick order. This does not include mild problems that may be genetic but will not affect the puppy’s life as a companion pet. A dog that has borderline/mild Hip Dysplasia can still lead a normal and active lifestyle and be a wonderful pet. The hip x-rays must be submitted to the Orthopedic Foundation for Animals for evaluation and a copy of the OFA results must be supplied to the breeder. In the event that the puppy/dog is determined to have severe heart conditions, the owner must have a complete cardiac exam preformed by a board certified cardiologist, which includes a chest x-ray, EKG and ultrasound. It is important to note that not every murmur heard in a puppy signifies disease. Some are innocent murmurs that dissipate as the puppy grows. A letter from the Cardiologist must be supplied to the breeder. The owner must also have the pup/dog spayed or neutered. The breeder shall not be responsible for any veterinarian expenses incurred buy the buyer. The breeder also reserves the right to have the puppy/dog examined to confirm the diagnosis. Breeder HAS NOT warranted or guaranteed to the Buyer that the Puppy will be free of genetic problems. However, in the event that the puppy should be diagnosed with severely crippling/life threatening genetic disorders which require surgery, such as hip dysplasia up to 6 months (24 weeks)

8. This puppy is being purchased as a companion animal (pet) with limited registration, which will prohibit breeding of the puppy and registering of the offspring. Unless breeder gives permission to breed.

9. The Breeder does not assume any liability for any injury to puppy after the puppy has left the Breeder’s premises, which includes; injuries caused by over exercise, weight gain, jumping, poor nutrition, dehydration, and excessive play. The Buyer agrees to keep the puppies exercise and jumping to a minimum until after 12 months of age. The Breeder can offer no further guarantee once the care of the puppy is no longer under the Breeder’s control. It is the sole responsibility of the Buyer to conform to the limits set forth in this contract.

We have read and understood the preceding paragraphs and by our signatures, we understand that we are bound to the preceding terms and conditions of this contract. It is expressly understood that if any of the clauses of this contract are breached, the remainder of the agreement will be considered null and void and the Breeder will have the right to repossess the puppy, the Buyer will be liable for any costs involved in that repossession. The Buyer will be responsible for breeder’s attorney fees, any and all court costs, which are to be paid at the time of the court’s decision. Any litigation concerning this contract shall take place in Hollidaysburg Court House. All information provided is true to the best of the Breeder/Seller’s knowledge and belief.

This contract conforms to the Pennsylvania Law on the sale of puppies.
